In this episode of the OneMDX videocast series, we sit down with Alex Thompson, Managing Director ANZ at Medline, to explore his unconventional journey into medtech leadership and the lessons learned across capital, interventional, orthopaedics, and consumables.

Alex shares how a career that began in IT evolved into senior leadership roles across some of the world’s most recognised healthcare organisations, and what ultimately drew him to Medline’s unique culture and customer-first mindset.

We discuss Medline’s remarkable global growth story, its scale across supply chain and manufacturing, and how the business is responding to shifting healthcare pressures around cost efficiency, data, sustainability, and customer expectations in a post-COVID landscape.

At the heart of the conversation is leadership and culture - from how Medline attracts and retains talent, to the principles Alex uses when hiring leaders and building high-performing teams.

Guest: AJ Moreland, Managing Director - Pacific Region for Dräger

For this episode, we sat down with AJ Moreland, Managing Director for the pacific region from Dräger , who started his career on the frontline in emergency care before moving into medtech.

That perspective really came through. From critical care and ventilation through to patient monitoring and hospital infrastructure, Dräger sits right at the centre of it, particularly during the COVID years where demand, pressure, and responsibility were at their peak.

We spoke about leading through that environment, the importance of honest feedback (even when it’s uncomfortable), and the reality that you don’t need to have all the answers from day one to succeed.

We touched on how in the bleakest of times, medtech companies rallied and supported eachother. A good reminder that, at its core, this industry works best when everything aligns around the patient.

Make no mistake, Telehealth platforms have been around for decades, it isn’t a new concept. We live in an age of convenience, we can order food at the touch of a button from the comfort of our front room, we can buy a week’s worth of groceries from the beach, and we can renew our passports or driving licences on the go, why shouldn’t we then be able skip the trek to the doctor’s office and instead just open a new window on our laptop, and sit back for a virtual visit with a doctor. Well we can, and despite the global challenges that have presented as a result of the COVID-19 pandemic, it has given rise to the emergence of Telehealth and the future of patient to surgeon/clinician interaction. Without a vaccine, physical distancing and aggressive testing have been the only tried-and-true methods to slow viral spread. That’s prompted an explosion in telehealth sessions to replace in-person care.

           What is Telehealth?

As defined by the American Telemedicine Association, “Telemedicine is the use of medical information exchanged from one site to another via electronic communications to improve a patient’s clinical health status”. Generally speaking, Telehealth or Telemedicine (the two terms are often used interchangeably) refers to the use of technology to deliver healthcare, and includes everything from video conferencing and remote patient monitoring to keeping and sharing electronic medical records. Telemedicine encompasses a growing number of applications and technologies including two-way live or streaming video, videoconferencing, store-and-forward imaging along with the internet, email, smart phones, wireless tools and other forms of telecommunication. Pretty Simple!

           COVID-19 & Telehealth

The Coronavirus has taught us many lessons and much of it the hard way. Those of us who are on the front-line around the world battling an invisible enemy are forging the path & we are indebted to them for the sacrifices they are making as they help us better understand the illness and look to develop vaccines and treatments to ensure our safety. We are learning to be more compassionate, understanding and sensitive to the plight of those around us and we have been given the opportunity to assess what is truly important. Other lessons are being imparted, too, especially as we employ technology in different or expanded ways. With social distancing, we’ve had to learn to live in a more virtual world – after work drinks are being done through a screen, sales presentations are being delivered electronically and healthcare is being delivered through a firewalled connection.

As our healthcare sector deals with the current COVID-19 situation, general practices are rapidly changing the way care is accessed and delivered to ensure the safety of their practice teams, patients and the broader community. One of these changes is increasing the ability for general practice and specialist consultations to be conducted via telehealth. Formal measures have been taken by the Australian Government to support healthcare practitioners to use telehealth, where appropriate, by introducing new Medicare Benefits Schedule item numbers, which is a huge step forward in bringing to the fore the benefits of Telmedicine.

More than 4.3 million health and medical services have now been delivered to a total of more than three million patients through the telehealth items introduced by the Australian Government for the COVID-19 pandemic. But increasingly, connected devices are part of the equation, enabling remote patient monitoring as well as communication. With connected devices, patients can provide their practitioners with metrics on, say, heart rate, temperature, lung function and more. There are a number of examples of where we have seen Tier 1 global medtech businesses invest in such platforms across a range of therapeutic areas including Orthopaedics, Diabetes and Cardiology pre-Covid and I would imagine this will be a trend we will continue to see

           What are the benefits?

The promise of telehealth emerged prior to the current health crisis, but its potential has come into focus as hospitals become overwhelmed with patients as well as to prevent clinicians falling prey to the virus as they treat the vulnerable. A key benefit for telemedicine is cost related. One study of chronically ill patients found that telehealth reduced emergency visits by half and hospitalisations by almost 90%.

Another key advantage of digital health sessions is the ability to provide care to more patients , often to vulnerable populations in rural areas who lack convenient hospital access. Given the vast distances between major cities and towns across Australia and the fact social distancing restrictions will likely be in place for some time, the benefits of telehealth services are clear. Furthermore, its capacity to contribute to data sets that can identify trends, enabling predictive analysis gives clinicians an advantge. Over the past decade, data analytics have been critical in understanding conditions such as diabetes and various cancers, the emergence of telehealth during this pandemic will likely lead to a broader application. For some time, big data has been used to understand risk factors as well as genetic predispositions of illnesses. Data collection of this nature may well mean that we are better prepared in the future should we encounter another Covid-19.

           The Future

Practitioners believe it is here to stay. While telehealth has been a popular concept for some time, COVID-19 has accelerated its push into the real world and the benefits, both economically and clinically are becoming clearer to identify. “We don’t adopt technologies to share patient-centric information, but we are quick—perhaps too quick—to adopt new technologies for clinical reasons. Now, all that is beginning to change,” observedDr Howard Luks, an orthopedic surgeon based in Westchester County, just outside of New York City.

COVID-19 will alter so many aspects of our lives in ways we haven’t realised quite yet, the hope is that these changes are for the better. We can be confident however that there will be fundamental changes to our healthcare sector and the way care is delivered and prescribed; Telemedicine will be a critical driver for that change.

Last week we announced our partnership with Latitude IT, a specialist recruitment business that offer placement and talent search services to digital & IT professionals. Alison Evans and James Gildon, co-founders of Latitude IT, are passionate recruitment leaders we have known for many years, they are true experts in their field and share many of the same values and philosophies that underpin our way of working. Few industries are able to demonstrate the true impact innovation and technology has on our lives quite like the MedTech sector. The demand for digital transformation skills within the healthcare space is at an all time high, COVID-19 notwithstanding, the “digital health” market has experienced steady growth over the last few years, with the recent increase in utilisation of telehealth/telemedicine platforms (which I wrote about last week), virtual reality and other AI based solutions, the decision to merge the knowledge and experience from both the commercial healthcare & IT sectors is one that will further benefit our clients and candidates.

We are really excited about our partnership with Latitude IT and the digital health space, but what does the future hold for this relatively new market?

Digital health is in its infancy and covers a large scope of products and treatments. It can cover everything from wearable gadgets to ingestible sensors, from mobile health apps to artificial intelligence, from robotic carers to electronic records. There are already more than 800 million users of digital health services globally and the objectives of this industry are diverse and complicated: preventing disease, helping patients monitor and manage chronic conditions, lowering the cost of healthcare provision, and making medicine more tailored to individual needs.

All of the major tech companies with a consumer presence are looking to get more deeply involved with digital health, but they’re all taking interestingly different paths. Google has probably the most comprehensive strategy when it comes to this space, they have a stake in the health tracking wearables market whilst at the same time pumping some serious resource into its artificial intelligence for healthcare offering through it’s “deep mind” unit. In addition, Alphabet — Google’s parent company — also oversees Verily, its health sciences division. Up until very recently Verily had a significant stake in Verb Surgical robotics and a well documented, yet rather clandestine collaboration with Johnson & Johnson. The collaboration was designed to bring together J&J’s surgical division Ethicon’s capabilities in surgery and advanced instrumentation with the Alphabet company’s expertise in machine learning and data analytics to build robot-assisted interventions for use in open surgery, laparoscopic, percutaneous and endoluminal procedures. Certainly an exciting prospect and an example of how the world of “tech” and healthcare are converging. Apple’s strategy is far more focussed on the here and now, expanding the functionality of the Apple-Watch to make it more health focussed by including ECG and falls detection capabilities, whilst Amazon I believe is ramping up its efforts to remake how healthcare is delivered in the US after acquiring a medication delivery business. You would have to be a brave man to bet against anything Mr Bezos touches!

There’s a number of reasons why tech companies are targeting health in particular: it’s a huge industry and generally speaking, it’s recession proof. As COVID-19 sweeps around the world, we are starting to see countries exit from lockdown, others introducing more stringent lockdown measures and others relying on the resilience of their health and care systems to allow them to focus on protecting their economies and minimising job losses. Health systems are however having to adopt to this strange new world, and the old ways of working, the tried and tested “office visit” seem rather now somewhat outdated and already out of step with how things will be done moving forward. COVID-19 will change the world in many ways, not least the delivery of care and the methods in which that care is provided.

The reliance on medical technology will increase at a pace and scale we have not seen before, the effect COVID-19 is currently having on the digital health sector is unprecedented, with a marked acceleration in the adoption of technology across healthcare systems around the world. My belief is that many of these technologies are here to stay and this market will only increase in size, we will continue to see a spike in demand for unique skillsets that straddle both commercial healthcare & digital transformation. Already we are seeing medical device professionals in Australia move across from clinical environments like Orthopaedics & Structural heart, to digital health start-ups offering unique ways to reduce the cost burden associated with emergency patient care. We will continue to witness this trend develop now and into the future as more digital health companies are born and a greater number of unique job’s are created.

Whatever the world looks like post C-19, my prediction is our healthcare system will look different, largely in part to the focus on a new and emerging market; Digital Health.

The federal Government of Australia this week announced the easing of lock down restrictions that have been in place across the country since March in attempt to not just “flatten the COVID-19 curve”, but virtually eradicate the virus. This weekend will see cafe’s, restaurants & pubs open their doors once again to the general public and is a sure fire sign that we as a nation are taking our first steps on the (bumpy?) road to recovery. No matter your politics and your views on Scott Morrison’s handling of the pandemic, Australia & New-Zealand are the envy of the world and there is an horizon, albeit a somewhat blurred one, in sight. As the economy slowly starts to kick back into gear and as we see a gradual return to offices across the country, I wanted to share my thoughts on how the Medical Device industry has rallied and responded over the last 6-8 weeks, how have Medical Device professionals been impacted & what does the rest of 2020 look like for our industry.

The Medical Device sector has been well and truly at the epicentre of the crisis. The COVID-19 pandemic has seen an increase in demand for all types of medical devices and some companies are seeing a huge surge in revenue as this demand continues to rise. Medical Device companies who manufacture and distribute ventilators, medical clothing, face masks, and diagnostic devices such as test kits are smashing their targets and are having difficulty keeping up with demand, whereas the revenue of companies, whose products rely heavily on elective and non-essential procedures, are expected to be impacted negatively now and into the future.

Many Tier 1 global device companies including Stryker, Smith & Nephew & Boston Scientific have revised their financial guidance for 2020 due to the uncertainty associated with the degree and duration of the impact of the outbreak, all three of these companies rely heavily on elective procedures as mentioned and the demand for their products so are perhaps more exposed than companies like Abbott or Roche who offer a diversified portfolio in areas which are seeing an increase in demand and activity as a direct result of the pandemic. The conversation’s I have had with clients and candidates over the last few week’s go someway to validate this theory in practice. What then is driving change and impacting our industry so broadly?

           Elective Surgery

Just this afternoon, Scott Morrison announced the resumption of elective surgery across Australia. “The boom is going up on elective surgery all around the country,” he said, following the complete removal of restrictions on Elective surgery in SA on Thursday 14th May, this news will be welcomed by patients who had their surgeries cancelled due to C-19 and those that work in the medtech space. Many of our clients and candidates have been sitting and waiting to see how things would be opened up, and whilst some specialties including Orthopaedics have seen some increase in activity over the last few weeks, up until now other areas, such as CMF, Bariatrics, ENT & Colorectal were still on a hard stop resulting in increased waitlists and frustration across the board.

We are all aware of the so called “second wave” and I have no doubt that the tap will be turned back off should we see a spike in numbers and infections. The general feeling is we have the resources and capacity to deal with an inevitable second wave and I am hopeful elective surgery and the work to make up for lost time will begin in earnest. For those of you that work in surgical sales, hold on to your hats, you are going to be busy!

           Economic Measures

Clearly, when there is an overnight downturn in revenues, companies will look to put measures in place that protect the livelihood of their employees and the viability of their business. I have said repeatedly for the last 8 weeks, if you work in medical devices and you haven’t been impacted economically, you can count yourself very lucky. It is reassuring that we are yet to see widespread, large scale redundancies as we have in other sectors, and I am hopeful we won’t. Whilst we have seen pockets of redundancies, most of those companies are now looking to re-hire those they let go, if they haven’t already. I am sure the combination of jobkeeper relief payment and the news around elective surgery has given these hiring managers confidence that good times will return and they wont be too far away.

It would seem that reduced working hours and enforced leave are the norm for most businesses with the impact ranging from 20% up to, in some circumstances, 80%. As much as this hurts the general feeling across the board is that people are simply thankful to have a job, and the hope is that these measures are short lived, simply put in place to navigate the uncertain waters and preserve cash. Teams I think are becoming closer, people are getting to know one another better (not least due to countless, and dare I say it (**meaningless**) zoom calls. This however, can only be a good thing, it is a shame that it has taken a global pandemic for many teams for feel more “connected”. Medical sales can be a lonely job, perhaps coming off the back of this it wont feel quite as lonely.

           Employment Market

Naturally, from a hiring perspective, things are generally pretty quiet, but it is without doubt getting busier. Some of the roles we were actively working on 2 months ago which were put on ice are now coming back, again likely due to renewed confidence around the reintroduction of elective surgery. Many Tier 1 global companies have indefinite hiring freezes in place, given the plight of other countries, in particular the US, the local branches of those businesses are often governed by the mother ship and until fortunes in other countries start to turn, I don’t expect to see those freeze’s thawing out anytime soon. Many companies however are continuing to speak with good talent in anticipation of a revival, which is the smart play. This past week I have seen more roles pop up across the market which is indicative of where we are at as a nation. I am hopeful we will continue to see things ramp up as we move forward into winter.

We have seen a surprising increase in enquiries from candidates, from those proactively looking for work to those who are keen to open up early discussions around the market and see what is out there. Despite the obvious challenges presented by C-19, this time is being used by many to assess their lives and with that, whether they are in a job and company that makes them happy. This results in a surge of good, quality candidates who are excited about the prospect of a move and are taking a proactive approach to seek advice from people like me about what the market is doing, who is hiring & what opportunities exist.

If you are a hiring manager, the message is simple; use this time to speak to talent. There has never been a better time to speak to good people. You have an active audience, you can dedicate time in an efficient way and people are keen to speak with good companies. Sure, you may not have a vacancy right now, you may not know when you will, but make those connections now, I can absolutely guarantee that your competitors are doing it. Use this time to determine what your business and team truly needs and you will be better equipped when we start to turn a corner. Sometimes it is easier to determine the cost of hiring someone than to determine the cost of NOT hiring someone, and in competitive areas like Ortho, Spine, & Cardiac, lost business through lack of resource is a very strong possibility.

           The rest of 2020

As has been the case since we all went into lockdown some 2 months ago, things are evolving on a daily basis and not least the outlook for the medtech sector across the remainder of the year. Given restrictions are to be eased this weekend and elective surgery is to be reintroduced, medical device leaders here in Australia will have a much better idea a month from now as to how the rest of 2020 will pan out. Capacity, case numbers and elective bookings are all essential metrics that these leaders will be looking into closely, and with baited breath, to decide to what strategies they put in play to tackle the back half of 2020 and beyond, it is perhaps still too early to predict.

I encourage everyone to remain optimistic, positive and hopeful. However bad the storm, there is always sun somewhere over the horizon.

From blazing fires that ravaged millions of acres across the country, to the Coronavirus or “COVID-19”, a pandemic that is affecting people and countries at an unprecedented level around the world, it is fair to say that 2020 will be a year many will sooner rather forget.

COVID-19 is an ever-evolving issue, with Governments around the world implementing measures that have significantly altered and changed the way we live our lives and interact with our families, colleagues, neighbours & friends. The outbreak, which has affected more than 500,000 people to date, has left businesses around the world counting costs. Industries from Travel & Tourism to Hospitality & Entertainment have been brought to their knees with huge job losses and in many instances reducing revenue generation to zero. Big shifts in the stock market have left many investors fearing that the continued spread of the virus could destroy economic growth and that government action may not be enough to slow the decline. In many countries; Central Banks have as a result slashed interest rates to encourage spending and make borrowing cheaper in order to mitigate the impact.

Healthcare systems in particular are feeling the strain and in some countries have been simply overwhelmed by the numbers. Doctors and Clinicians are now having to make life and death decisions based on what resources they have available; there are only so many ICU beds & ventilators available and the demand far outweighs the supply. It has been well documented how Healthcare workers on the frontline are running short on critical PPE (Personal Protection Equipment). Without these essential supplies, containing the virus and ensuring the safety of our Nurses and Doctors cannot be guaranteed; we need these individuals now more than ever and it is pleasing and reassuring to hear that production levels of these essential supplies is ramping up considerably.

Despite the doom & gloom, there are some positives to take from COVID-19. Families and communities are more connected, pollution is down, employees can be productive, in some instances even more so, when working from home & we ultimately have a greater appreciation for our fundamental freedoms which we have taken for granted for so many years. A personal highlight for me has been witnessing organisations such as Tesla and Dyson shift away from their core business focus to positively contribute to the fight.

We at OneMD have been keeping a keen eye on how MedTech businesses have been responding to the crisis and it makes us proud to be associated with an industry that is doing everything it can to deliver care to patients who need it most. The fundamental mission of Healthcare companies is to advance patient health, and in times of emergency, this goal stands high above all others. Nobody can know what COVID-19 will ultimately mean for global health, affected societies, or the global economy. But Pharmaceutical and MedTech companies are critical parts of mitigating its impact and coming up with a global solution.

We have spent time speaking with Gavin Fox-Smith, previously Managing Director of Johnson & Johnson ANZ & Chair of the MTAA and now Chair of ANDHealth (Australia’s National Digital Health initiative) and Co-Chair of the A/NZ Leadership Forum Health Technology Group to understand the impact of COVID-19 on the Medical Device industry and put to him the following questions:

           1. What can senior business leaders in the medtech space do to ensure that their people are staying focussed, committed and happy during these times?

My first thought is fundamental to our industry – remember why we exist, which is for the patient. In all circumstances once the patient is at the centre its amazing what magic happens. Equally important is our clinical customer, none more so than at this time. Whether it is a surgeon, physician, GP, nurse, infection control or allied health they are relying on our industry to serve and support them at this very challenging time.

Whilst our primary focus is rightly on the patient and customer we clearly need to look after our people (ie employees) and their families. Paramount is their own health (not just Covid-19 but their broader physical and mental health). We need to ensure their working environment is safe and productive (especially those such as manufacturing or warehouse staff who still need to attend a work place). The work life balance has never been more important. We need to ensure constant, consistent and transparent communication – both out and in, people must be encouraged to raise any concerns without fear. In the bigger picture every business will want to keep strong and sustainable so their business is still here in the post Covid-19 world.

Finally our industry has always led from the front in community responsibility, never a better time than now to keep demonstrating our care for the community we are so privileged to live and work in.

           2. Many people who work in medtech sit in client facing functions; what advice do you give to those people in order to remain productive?

We have grown up in a time where our industry has defined client facing as “feet on the ground” or “reps in the OR” or “hands on demos”, which has served our customers and our businesses well. Given the available technology I think it would reasonable to say we have not embraced it to the level we could have. We can now do this at pace and scale. Field based people have always had account/sales/territory plans – I see no reason to change these with the exception of how we connect. It is a critical time for our first line leaders (ie Sales and Business Managers) to be engaging with their respective teams even more regularly. And you can still have fun with many examples of morning coffees or Friday drinks over Zoom or similar technologies!

Even with this our teams will have more time available than normal – what a great opportunity to build skills, learn new things, develop yourself. Education should be a primary productivity tool over the coming weeks and months. Those businesses that are the most ready for a post Covid-19 environment will thrive.

           3. How do you see this crisis changing the way Medical device organisations operate/interact with their customers ?

I think this is the key question for all industries and all organisations. One thing is certain – the pandemic will change the way everything is done in the future. At the enterprise level Boards and Senior Executives will be critically examining their business continuity plans. Those businesses that have strong cash flow, diverse revenue streams and top talent will not only get through this challenge well but will accelerate post Covid-19.

We have seen the widespread adoption of digital technologies for Healthcare professionals and providors.I think we will see this trend continue to the point where many of our companies will need to re-think their customer engagement and revenue demand activities.

           4. In what ways can medical device companies positively contribute to the fight against COVID-19

Medical device companies are contributing in a multitude of ways across many different fronts. Firstly our industry has come together under the banner of Medical Technology Association of Australia (MTAA), both members and non-members, with direct line to both Federal and State Departments. Focus areas are Ventilators, Testing Kits and Personal Protective Equipment (PPE).

The major global players (and even now companies like Ford and General Motors) are mobilizing at a scale never seen before. Equally we are seeing our local Australian owned businesses step up, a great example being Med-Con in Shepparton. They even have the Army helping them scale up production of medical masks!

           5. Naturally, we know that there is an end point to COVID019. When we reach that end point, how do you feel the industry will respond?

Our great industry has innovated and responded to patient needs for decades, and we are seeing the best of our industry as I write this. I have enormous confidence in our industry and its ability to adapt and respond to all challenges – we will get through this and we will be better and stronger as an industry!

Thank you to Gavin for contributing his thoughts on an incredibly important topic for anyone connected to the medical device sector.

The reality of life post-COVID-19 has not fully sunk in yet, and its consequences for our businesses, organizations, economy, and society will play out over the rest of 2020 and beyond. We encourage everyone to remain positive, optimistic and resilient, good times will return.

Counter offers are rising- this guide helps candidates weigh your value, motivations and risks so you can decide if staying or moving is the right move.

In today’s current recruitment market, counter offers are more frequent than ever! In some ways, this is great way for your company to show you how much you mean to them and are valued and in others can mean dealing with conflict and being thrown into a world of turmoil about “the right thing to do”

Here at OneMD, we have seen, first hand, these situations arise and while each scenario is unique to each individual, we have a general guide to assist you in navigating these situations and ultimately making the right decision for you!

Reasons for the Counter offer from the employers perspective:

It can be expensive to replace an employee (e.g. recruitment costs, loss of productivity from the team, loss of working hours, training costs and advertising fees). It is often easier and more cost effective to increase the salary for the employee who has resigned

Uncertainty of the quality of a replacement ( it’s always risky to take on someone new and although interviewing processes are used to qualify a new person, there’s no guarantee of their abilities/suitability until they actually start)

They may have known that the employee was worth more to the company for a long time, but felt they were able to maintain them within the business under the current conditions – only after a resignation is made does the company decide to act upon this

One leaver can sometimes lead to more (e.g. others in the team realise what is available to them too, the leaver is a prominent figure in the team and the dynamic would be affected)

We estimate that at least 90% of candidates who accept a counter offer will leave their current employers within 6 months and 94% within 18 months

What to consider when a counter offer is offered:

What were your original reasons when applying for the other job?

How high was your current level of job satisfaction? You must have had motivation to move.

Will the counter offer negate these motivations?

Why did it take your resignation for your employer to offer an improved salary and benefits package?

Will you need to threaten resignation every time you’re due a raise

Counter offers can be verbal promises; there’s nothing to stop your current employer from changing their mind as soon as you formally reject your external offer. This happens more frequently than you may think.

Ultimately counter offers can be great but also be counterproductive. This has been brought to light more and more by Covid and how employees have either been treated by their employers or by the lack of progression available to them
